- Download and print the
Medical Questionnaire and
Application form.
These are in PDF format. You can obtain
Acrobat Reader (free), which will allow you to view and print the forms, from Adobe.
The Medical Questionnaire is intended to determine if you have developed any
medical condition since your initial training that might even remotely cause a
problem while using SCUBA. If any possible conditions exist, you should contact the
instructor to discuss the condition. You may need to obtain a doctor's approval
before registering for the course.
- Complete the two forms and send to the address provided on the registration form.
Once the instructor has reviewed the forms you will contacted to setup up a schedule
for the various sessions.

Review diving theory and concepts on the Internet and
complete the associated quizzes.

| Classroom reviews |
One or two class sessions will briefly cover all key
areas with emphasis on diving procedures and quiz questions answered incorrectly.

One or two pool review sessions based on your need.

Group Fees for 2 or more:
- $35 - Registration & Lecture Review
- $15 - Pool (per session)
- $45 - Open Water Dive (includes quarry entry fee)
